AMC Health has developed MyCare with patients like you in mind. To utilize this service, prior enrollment as an AMC Health monitored patient is required.

MyCare is designed to assist you in maintaining your well-being for a healthier lifestyle. The MyCare app is compatible with a range of AMC Health devices, including the Pulse Oximeter, Thermometer, Weight Scale, Inhaler Sensors, and Blood Pressure devices. If you own your own home health equipment, the MyCare app offers a convenient and adaptable platform enabling you to input your health data using those devices.

In addition to facilitating easy data input, we provide access to various tools that allow you to track the trends of your vital signs on a daily basis, spanning from 7 to 30 days. Your data is securely stored in a centralized location for effortless retrieval, presented in clear charts and summaries. This enables you to gain valuable insights, acquire knowledge, and adjust your lifestyle as necessary. You can even receive nearly real-time feedback to support the achievement of your individual health objectives.

The MyCare app includes alert features as well as support and guidance services. Whether you are at home or on the move, your clinical team or health coach is available to assist you in managing chronic conditions such as diabetes, heart failure, COPD, hypertension, and weight management.

If you require reminders to adhere to your care plan, the MyCare app provides timely assistance. You have the option to establish reminders yourself or have them set by your clinical team (nurse, doctor, health coach), ensuring you remain connected precisely when and where it is most beneficial.

For enhanced convenience and security, biometric login options are offered along with easy access to various tools including two-way messaging functionalities.

Patients who have activated video capability within CareConsole can engage in Nurse-initiated Video Calls via the mobile app.

Furthermore, patients can now access educational video content through the app to enhance their knowledge of self-care practices for their specific conditions.
